Who can Refer a child
to ECI?
In most cases a child is referred by his or her parents or by the child's
health care provider. However, anyone can refer a child to ECI.
How do I refer a child
to ECI?
You can:

Telephone ECI directly at the number listed above

Download and print the ECI Referral form, fill it out, and send it to the
address or fax number listed on the bottom of the Referral Form.
In order to keep your child's information confidential we recommend that
you use one of the two methods listed above. At this time we do not
accept Referral Forms sent via email. We are happy to answer your
non-confidential questions by email at info@eci-lps.org.
